Medical records play an important role in supporting the achievement of orderly administration in hospital health services. The success of the implementation of the Raja Ahmad Tabib Regional General Hospital in the Kepulauan Riau Province is related to the implementation of medical records. However, in its implementation there are still obstacles such as the lack of cooperation between the patient and the medical record staff in filling in the identity, as well as misplaced and multiple patient statuses. Therefore, so that this can be minimized, it is necessary to improve efforts that look at the management of medical record personnel by taking into account several indicators of Total Quality Management. This study used a qualitative descriptive method, with the object of research being five medical record personnel in the ER registration section, four medical record personnel in the filling division at the Raja Ahmad Tabib Regional General Hospital, Kepulauan Riau Province, and the Head of the Medical Record Room. The results show that several indicators have been implemented well, but there are still some that need attention, starting from deepening effective communication training in order to create cooperation in filling out identity data between the patient's family and medical record personnel, as well as communication in equipment so that it is in accordance with the needs of the patient's family. work, as well as deepening SOPs so that carelessness in placing patient status, as well as dual status can be minimized.
